House Calls is all about you

During a House visit, a licensed health care practitioner will visit you in your home. It’s an opportunity to ask questions and get answers about the things that matter most to you about your health.

Every in-home visit includes:

• Up to an hour of 1-on-1 time with a health care practitioner

• A physical exam

• Health care screenings tailored to you

• A medication review

• A chance to get advice and ask questions about your health 

• Education, prevention tips and referrals to health services, if needed​​
